All of our arrangements begin with gathering the information needed to complete Ontario’s Form 15, the official Statement of Death. This information becomes part of the province’s permanent record and allows us to prepare the legal documents, including Proof of Death Certificates and government benefit notifications, your family may require. This process also helps ensure that government programs are updated properly and that everything is handled accurately and respectfully. By providing these details now, you allow us to take care of the necessary steps on your estate’s behalf.

Data collected is used for funeral related documentation and applications for related funeral benefits. Information collected is not sold to third parties. As most boxes will require information to be included, when you do not know the answer to the question, please enter “unknown”.
